Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed by Cavalieri Psychiatry, and how you can get access to this information. Please review it carefully.

Our Commitment

We are required by law to maintain the privacy of your protected health information (PHI) and to provide you with this notice of our legal duties and privacy practices. Psychiatric records may receive additional protection under federal and state law.

How We May Use and Disclose Information

We may use and disclose your PHI for treatment, payment, and healthcare operations, and as otherwise permitted or required by law. Most other uses and disclosures — including most disclosures of psychotherapy notes — require your written authorization.

Your Rights

You have the right to inspect and obtain a copy of your records, to request corrections, to request restrictions on certain uses and disclosures, to request confidential communications, and to receive an accounting of certain disclosures. You also have the right to a paper copy of this notice upon request.

Questions and Complaints

If you believe your privacy rights have been violated, you may contact the practice directly at (727) 476-7778. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be penalized for filing a complaint.
